All requests for access including new requests or to extend access must be on a User Access Form (see exceptions below). A User Access Form is not complete until it has been returned with Manager’s approval. If you are not sure if you need to fill in a new form, get in touch by emailing interrai@tas.health.nz or calling 0800 10 80 44 (option 3).
User Access Form
Click here to complete the online User Access Form to request training and/or to update your interRAI Assessment Software (iAS) access.
The User Access Form can be completed on a mobile, tablet or computer. A copy of the data submitted is automatically sent to the person completing the form and to their nominated manager for approval, please ensure you input email addresses accurately.
To complete the form, you will need to have the following information ready, as the form can't be saved until it is complete:
- Your contact details, including personal email address and mobile number (required for the interRAI Learning Management System)
- Your workplace details
- Your manager's name, work phone number and work email address
- Registered health professionals: your registration or Annual Practising Certificate (APC) number
Removing access
Access is removed when:
- We are informed by the user, or their manager, that they have left the organisation or no longer require access. Send an email to interrai@tas.health.nz and include the date that access is no longer required from.
- We are advised that a person has changed jobs. A user access form is only valid for the role which it was requested and approved for. If you move roles, you will need to complete a new user access form.
- The system hasn’t been accessed for more than 6 months. A new user form will be required to gain access again.
Extended or parental leave
If you are aware that you are going to be on extended leave but intending to return to your role, please email interrai@tas.health.nz and let us know, including your expected return date. We will hold your access until your return.
Exceptions to completing a new user form
- You informed us of extended or parental leave and are intending to return to work. Our Education and Support Team will check to see if you need to complete any competency requirements and your access to complete assessments can be reinstated.
- We have a user form for you that states you are approved for access to all the ARC facilities in your region and a new one has opened in your area. Email us and we can add this on for you.
